The design adds 74 resident rooms in two skilled nursing wings, and an additional wing for six hospice care suites. A new entrance opens to the services, shops, dining and social activities along the new Main Street addition, creating a welcoming destination for residents within the larger campus. The Main Street facades are inspired by the traditional downtown architecture found throughout South Dakota's rural communities. 
Rehabilitation therapy, administrative spaces, and common dining areas of the existing wings were renovated as part of the overall project.
